An advanced care team supports your primary care physician in their efforts to deliver effective patient centered care. An advanced care team can be made up of registered nurses who act as health coaches, pharmacy technicians, pharmacists, licensed clinical social workers, psychologists, psychiatrists, nutritionists, dietitians, case managers and care coordinators. These people make up a team that assists your physician in taking care of you.
Joy assists patients in obtaining prior authorizations on their medications when insurance companies require it. She also assists patients in locating the best price on their medication or finding savings cards for them. She is also available to assist patients in understanding their medication and the purpose for it.
Dr. Samuel Thielman is available for psychiatric consults for a wide variety of issues. Dr. Thielman provides medication management, as well as, counseling and therapy. If the primary care physician and the patient decide that seeing Dr. Thielman would be a good idea, we will put a Co-Management Agreement in place.
A co-management agreement is an agreement between the primary care physician and a behavioral health provider. The agreement will typically spell out the responsibilities of each provider. For example a typical agreement would say that Dr. Thielman will agree to see the patient for a specified medical need and provide care around that diagnosis. It might also state that he would prescribe medication for that diagnosis and provide the primary care physician with regular updates on the patient’s progress regarding that diagnosis. The agreement would also indicate if Dr. Thielman will provide ongoing for this diagnosis or if he will refer the patient back to the primary care physician when the patient is stable. The purpose of the co-management agreement is to specify exactly what the primary care provider and the behavioral health provider is responsible for. The benefit to the practice is that it is clear what each provider’s role is in the care of the patient. The benefit to the patient is the same.
